➡️ What is the Control Group Health Card?   The Control Group Health Card is an “In Case of Emergency” card that you keep on you at all times. If you are ever found unconscious or unable to communicate, the QR code can be scanned to show important information about you, your health and your advanced medical decisions.   You choose which information you would like to show on the QR page – for example:   · Your blood-type · Any allergies · Medical conditions · Emergency contacts · Advanced decisions you have made about things such as; blood transfusions, organ donation and treatments.   All participants in the Control Group study can create a digital Health card – subscribing Associates can order a durable, plastic card to keep on their person as they travel day-to-day.   👪Cards can be created for you and your household. 🚫You can disable the QR code if you lose the card, or no longer wish to use it. ✅Details can be reviewed regularly and updated as necessary.   ➡️ LEARN MORE: https://controlgroup.coop/hc.html   ➡️ LOGIN: https://prod.controlgroup.coop/account/login

🩸 Remove Blood Group from Health Card Do you want to remove the picture of your blood group from your Control Group Health Card? Not a problem!   When you create a new Health Card, you now have the option to switch off the blood group – this only applies to any new cards that you create and not an existing one. Instead of your blood group, the Control Group logo will be displayed on your card. 📌 HOW TO RECORD YOUR CHILDHOOD VACCINES We have made it easy for you to record your vaccine status by creating a ‘Childhood Vaccinations’ option.
